Alfresco Dinner at Clearwater Resort Ticket Order Form

Please fill up and submit this form to order your tickets for the Alfresco Dinner at Clearwater Resort to be held on 1st December, 2012.

For the price of php500/adult and php200/child-12-year-old-or-under, participants will be admitted to Clearwater Resort starting 2:30pm. An outdoor dinner will be served at the Café Picnic Garden starting at 5:30pm ending at 8:00pm with full view of the lake. Some special activities will be organized and announced on the day.
